Located on Thompson Peak Parkway, Dr. Lawrence Cook has had over 40 years of medical experience. His small practice allows him to focus individually on his patients' needs. Affordable Care Act

Has the Affordable Care Act affected you? The new healthcare reform act might have affected your coverage, your deductibles, and your coinsurance. You also can find out if you are eligible for tax credits for private insurance or health programs like the Children’s Health Insurance Program (CHIP).

The next Open Enrollment period runs November 15, 2018–February 15, 2019 for coverage that begins as early as January 1, 2019.  Check you plans carefully.

Many plans both ACA as well as private or employer sponsored have such a high deducible that you really should consider them as catastrophic insurance rather than insurance that you can really use.

You may find that the amount you have to pay out of pocket has gone up considerably as the insurance companies adjust to shifting costs.

Online Forms

Cardiologists of Scottsdale has downloadable forms on it's website. Feel free to print them before your first appointment and fill them out. You can either scan or fax the forms to Dr. Cook's office or fill them out and carry them to your next appointment.

We are accepting new patients, though not all insurance carriers.  Dr. Cook rounds at the three Scottsdale HealthCare (SHC) hospitals, which have now merged with the John C Lincoln Hospital system. Collectively they are known as HonorHealth. Dr Cook's primary hospital is HonorHealth Thompson Peak Campus. You may request to see him as your cardiologist at the hospital.  If you would like to be seen in the office for your cardiac issues, please call  Cardiologists of Scottsdale.
