Retirement:

Dr. Cook retired and Cardiologists of Scottsdale closed on January 1, 2022. The last appointments were scheduled Nov 18, 2021.

We will not be able to continue to refill prescriptions after the office closes. You will need to find a different physician to continue your medications. For many, the Primary Care Physician may be able to continue your chronic established therapy. You will need to ask. Please don’t wait till your supply of medication is exhausted.

We are still working on our long term record maintenance so we can seamlessly send (fax) your records to the Doctor(s) of your choice. Records will be maintained for 6 years after the office closes per A.R.S. § 12-2297.

Transfer of records will be free of charge. There will be a fee for records not sent directly to your health care provider.
